Transcripts can be requested for appeal and non-appeal purposes. Expedited transcripts can be requested, but the final delivery date and price for expedited requests will be determined by the court reporter or outside vendor assigned to complete your transcript ("Transcriber").

After you submit your Order, it will be assigned to the appropriate Transcriber. The estimated cost for the transcripts will be based on the length of the proceedings requested. You will have an opportunity to review and approve the estimated cost. You are not obligated to proceed with the Order until you accept the estimate.

You may be required to pay a deposit prior to the commencement of transcription. Any delay in paying the deposit will impact the expected delivery date. You will be able to pay the deposit and any final balance online or by check. The estimate you receive is an estimate only. The final cost will be determined by the actual transcript page count. You will be responsible for any remaining balance. If a refund is due, it will be issued to the credit card used to make the deposit or by check.

Select the preferred date that you would like to receive the transcript. Please note that the actual delivery date depends on various factors and will be communicated to you by the Provider prior to the production of the transcript starting.

The Provider will also communicate to you a cost estimate of the transcript. The estimated cost of your requested transcript will be calculated based on the information you have provided, including the turnaround time you choose (see table below). You will be able to review the adjusted estimated cost prior to acceptance and the work commencing.

The final cost of your completed transcript(s) will be based on the total number of typed pages actually produced by the Provider. The final cost is calculated by multiplying the number of pages produced, by the applicable published page rate for Hendricks County, Indiana.

If any part of your request has been previously transcribed, those pages may be available at the Copy price listed above. The Court notifies you if this is the case and will adjust your cost estimate accordingly.

Juvenile Court Proceedings - Special Requirements

Legal Acknowledgment Required

Pursuant to O.C.G.A. §§ 15-11-704, 15-11-708, and 15-11-630(f), I acknowledge that if my case is NOT an appeal of an Adjudication/Disposition Order, Termination of Parental Rights Order, Order transferring a delinquency case to Superior Court, or Order denying petition waiving parental notification of abortion after a timely filed Notice of Appeal, then a transcript is not mandated by law and the Court retains discretion to approve or deny this request.

If the Court approves a transcript in a case listed above, I will not be charged for the transcript if I have been determined to be an indigent. However, if the Court approves a transcript in a case NOT listed above, I may be required to pay for the transcript even if I have ben determined to be indigent. Prior to the release of a transcript from a Termination of Parental Rights case for appellate purposes, the Court may reassess my indigency status.

If the Court approves the transcript, payment for the transcript will be due upon approval. I must contact the court reporter for transcript costs and payment arrangements. Requests for the expedited preparation of transcripts wlill incur additional transcript fees.

To request a transcript, I must complete and submit this form to the Juvenile Court Judge's Office assigned to my case. If approved, the Judge's Office will forward my request to the court reporter.

ORDER

Wherefore, it appears a transcript of these proceedings has been requesteed pursuant to O.C.G.A. §§ 15-11-704, 15-11-708, and 15-11-630(f). The Court hereby approves / denies release of the requested transcript. Any transcript released by the Court shall be used for Juvenile Court and Appellate Court proceedings only and is not to be disseminated in violation of O.C.G.A §§ 15-11-704, 15-11-708, and 15-11-630(f). Any dissemination in violation of this Order is prohibited and punishable by Contempt of Court.
